Please use the form below to make changes to your membership such as freezing, un-freezing, terminating, or updating your contact and payment info; these requests can not be handled over the phone. Please list all of the member(s) or participant(s) requesting the change.

Please note, all membership changes must be submitted prior to the 1st of the month for the following billing cycle.
